Health Insurance Plan and Policy
General Information:
Health insurance is required for all F-1 international students at Concordia. All F-1 international students will be enrolled in Concordia's health insurance plan according to the health insurance policy below. Beginning in Fall Semester 2019, Concordia's health insurance plan will be through LewerMark Student Insurance.
Students will be enrolled in Concordia's health insurance plan during required semesters/terms. Fall, Spring, and Summer are required terms for graduate students, so graduate students will be enrolled in Concordia's health insurance plan during these terms. Fall and Spring are required terms for undergraduate students, so undergraduate students will be enrolled in Concordia's health insurance plan during these terms. Summer is an optional class term for undergraduate students, so undergraduates should contact ISS if they would like to enroll in CSP's health plan for summer.

Student athletes will be waived out of the CSP health insurance plan as it does not cover collegiate sports. Please confirm with CSP Athletics if you have questions about the appropriate level of health insurance coverage. Ted Tryzynka (trzynka@csp.edu) is the Athletics contact for athlete health insurance requirement questions.
"F-1 students who do not submit waiver-eligible health insurance information by noon on the Friday of the first week of classes will be automatically enrolled in health insurance each term. Waivers received after the deadline will be applied to the following term. The cost of health insurance is included in your I-20 estimate and will be billed to your student account each term. If you become waiver-eligible in a future term, you MUST submit your health insurance waiver card to confirm. Your waiver will be applied to the following term. Likewise, if you lose your waiver-eligible coverage, you MUST contact ISS in order to be included in CSP's health insurance plan for the next term.
The following F-1 students are eligible for a health insurance waiver. (Note: A waiver means you will not be enrolled in CSP's plan.)
Waiver-eligible students:
-Saudi Arabian Cultural Mission (SACM) students
-Students with US employer-provided health insurance (from CPT, spouse, or parent)
-Exchange or select partner students: SoonChunHyang University, United International College, and InfoLink
-Student athletes (excludes eSports)
Concordia cannot accept individually purchased plans for the insurance waiver. For example, the following companies cannot be accepted for the insurance waiver: GBG, HCC, IMG, ISI, ISO, ISP, PGH, PSI.
Students with a visa status other than F-1 are also expected to carry a quality health insurance plan during their time at Concordia. Additional visa types covered under the LewerMark plan include J, Q, M, U, T, R, A, TPS, & DACA Students. If you would like to sign up for the LewerMark plan, please contact ISS."

- To access your ID card, visit CSP's LewerMark page. Your username is your CSP L number with a 0 for the L (ex. 000123456). Your password is your date of birth (in format MMDDYYYY). Then you will be prompted to set up your account.

- Note: This plan includes a wide network of care providers including the Minute Clinics at CVS and Target stores which has no copay for covered services with your health insurance card.
- Note: This plan includes health coverage but does not include vision or dental coverage. Please contact LewerMark directly if you would like to enroll in vision or dental coverage.

- If you are in need of a plan, the option below includes appropriate collegiate sports coverage per Athletics and may be purchased individually.
- eSports students are excluded from the student athlete waiver as eSports is not under athletics or regulated by the NCAA. eSports students will be enrolled in CSP's health insurance plan.
- It is your responsibility to opt-in to health insurance if you previously had a waiver or were on Leave of Absence but need CSP health insurance now.
- If you are a student who did not have CSP health insurance the previous semester but will need it in the current semester, please fill out the "Continuing Student Health Insurance Enrollment" request here.
- All new and continuing students will automatically be enrolled into health insurance unless a waiver was submitted by the waiver deadline.
